Specialist complex dementia care

A person-centred approach to dementia care
Our approach is personalised to the individuals that we support. We acknowledge and uphold the significance of personhood, and work with each individual, their loved ones and professionals to design a care plan that’s tailored to their needs and goals. Our care recognises people’s individual spiritual, emotional and physical needs. And we take time to understand each person’s life story and empower them to continue to do the things that they enjoy for as long as they’re able to do so.
Support for families and friends
At Exemplar Health Care, we understand that dementia doesn’t just affect the individual – it can have a big impact on people’s families and friends too, especially for those living with early/young onset dementia.We support and work with the families of people living in our homes, where they wish. Where appropriate, we include family members and friends in all aspects of home life – from moving in, to events and celebrations, and care planning - we know that loved ones can provide valuable ideas, care advice and emotional support.
We often involve families in training sessions to help them better understand their loved one’s condition and how to support them.
